Chronic Disease

GOAL:  Improve the education, prevention, detection and treatment of chronic diseases in conjunction with Measure Up Marion.

OBJECTIVE 1:  Identify underserved segments of our population within Marion County and develop chronic disease prevention strategies to improve diabetic health outcomes.

  • Action Step 2: Extract data from the Marion County Needs Assessment and other data services to identify underserved areas of Marion County

OBJECTIVE 2:  Increase education and awareness of chronic diseases in Marion County.

Action Step 1: Design a program to educate Pastors and Parish Nurse/health ministers on the goals of Marion County Health Alliance.
